Practice Reviews

The Single Unified Safeguarding Review (SUSR) Process 

The Single Unified Safeguarding Review (SUSR) process was officially implemented on 1st October 2024.

The SUSR aims to create a single Review process which incorporates a multi-agency approach where the criteria for one or more of the following reviews is met:

  • Adult Practice Review (whether concise or extended)
  • Child Practice Review (whether concise or extended)
  • Domestic Abuse Related Death Review
  • Mental Health Homicide Review
  • Offensive Weapons Homicide Review

Professionals who wish to refer a case for consideration of a Practice Review are asked to complete the appropriate Practice Review Referral Form below and return to the Safeguarding Board Business Unit via email ctmsafeguarding@rctcbc.gov.uk
